Provides skilled care, including administering medications, changing dressings, and managing catheters. A visiting nurse evaluates the patient's condition, takes vitals, and documents detailed notes on their progress. 

Delivers direct, individualized nursing care to patients under the supervision of an RN, ensuring safe and high-quality home health services. Implements care plans and maintains accurate, timely documentation. 

Prepares, implements, and documents a therapeutic plan based on the initial evaluation and the total client plan of care. Maintains communication with the RN and the family to stay informed of any changes in the client’s clinical condition. 

A physical therapist assists with overall mobility, including walking, getting in and out of bed, sitting, and standing. 

An occupational therapist assists with activities of daily living and mobility. They can also help relieve pain, improve range of motion, and build up and retain muscle performance. 

Speech therapists provide care to improve speech, language, cognition, voice, and swallowing. They may use drills and activities to enhance these skills or provide strengthening exercises for the muscles of the lips and tongue. 

Works under the direction of the RN Supervisor to assist clients with personal care activities, including bathing, toileting, and grooming.
